Update Your Information

Keep your personal information current to ensure that your benefit coverage is correct and that you continue to receive important information from SCPMG.  Submit an update request if you have a change to any of the following:

•  Address
•  Phone number
•  Legal name
•  Marital status
•  Emergency contact

To submit an update, contact PHR Shared Services and advise of your new information.   Depending on the update, you may be asked to submit additional supporting documentation.
