Survivor Benefits

To report the death of a physician, physician’s spouse/domestic partner, and/or dependent:

  1. Contact PHR Shared Servicess at 1-877-608-0044 or PHRSharedServices@kp.org
  2. Common Plan – upon the death of a physician, contact the Kaiser Permanente Retirement Center (KPRC) at 1-800-721-3647
  3. Keogh & 401 (k) Plan – upon the death of a physician, beneficiaries should contact the Schwab Retirement Plan Services Company at 1-888-256-8830 to determine if there are any active accounts for the Keogh and/or 401 (k)
